Scholarship Application Forms & Directions
Before submitting a scholarship application to SCS, please ensure each page is filled out entirely, signed, and dated. Incomplete applications will be returned.
Directions for completing and submitting forms:
- Enter only information for student applying for scholarship on the two consent forms (pages 2 & 3 of application); please do not include
- siblings who are not transferring into your school
- preschoolers
- Include all siblings on the Household Economic Survey (page 4 of application) even if siblings are not applying for scholarship.
- Before submitting application to SCS, please ensure each page is filled out entirely, signed and dated. Incomplete applications will be returned.
